Diabetes Center of Excellence
The Grady Memorial Hospital Diabetes Center of Excellence is ranked fourth out of
twelve affiliated diabetes centers statewide in total number of individuals completing the
program. The GMH Diabetes Center of Excellence, an American Diabetes Association
(ADA) recognized program, provides service to individuals with diabetes on both an
inpatient and outpatient basis.

The Diabetes team, consisting of referring physicians, along with registered nurses and
registered dietitians, provides American Diabetes Association criteria-based education and
self-management training in group and individual settings.

Individualized education and self-management training is offered in all aspects of diabetes
self-care, including: disease process * nutrition * physical activity medications/pills/insulin
* monitoring * crisis intervention * preventing, detecting and referring individuals with
complications * emotional/lifestyle changes * diabetes and pregnancy * gestational diabetes.
For the diabetic patient, education and self-management is the key to long term success and
control of this disease.

In the most recent 12 month period, 128 individuals completed the GMH self-management
training and education program. In addition, two support groups involving more than 242
individuals met on a monthly basis alternating between Educational and Living with
Diabetes topics. Ninety inpatients were also served with diabetes intervention to help reduce
their length of hospital stay.
