6. To expect all communications and other records pertaining to his/her care, including the
source of payment, to be treated as confidential.
7. To request another room based on a justifiable cause and to be informed of intended
discharge or transfer.
Patients have the right to expect reasonable safety insofar as Hospital practices and
environment are concerned.
Patients have the right to know the identity, professional status, and professional relationships
of all individuals involved in his/her plan of care. Participation by patients in clinical training
programs or in gathering of data for research purposes is voluntary.
Patients have the right to obtain from the physician complete and current information concerning
the diagnosis, planned treatment, and known prognosis. The patient has the right to participate
in treatment decisions.
If the patient has been adjudicated incompetent in accordance with law, is found by his/her
physician to be medically incapable of understanding the proposed treatment or procedure, is
unable to communicate his/her wishes regarding treatment, or is a minor, these delineated rights
transfer to the patient's guardian, next of kin, or legally authorized responsible person, to the extent
permitted by law.
Patients have the right to informed participation in decisions involving their health care. This
includes the right to a clear, concise explanation of any condition, all proposed technical
procedures, and risks of mortality or serious side effects, problems related to recovery, the
probability of success, the alternatives for care or treatment, and who is responsible for authorizing
and performing any procedure or treatment.
Should consideration of ethical issues arise in the care of the patient, the patient or his/her
designated representative has the right to participate in discussions of these issues with his/her
physician and/or the Hospital staff. If these issues are unresolved, the patient or his/her designated
representative may refuse treatment to the extent permitted by law.
The relationship with any patient may be terminated with reasonable notice when a refusal of
treatment by the patient or his legally authorized representative prevents the provision of appropriate
care that is consistent with professional standards.
Patients have the right to a complete explanation of the need for transfer to any other facility and the
alternative courses of action.
Patients have the right to be informed by the physician responsible for his care, or by a physician
delegate, of any continuing health care requirements following discharge from the Hospital.
Patients have the right to request and receive an itemized and detailed explanation of their total
bill for services rendered.
Patients have a right to timely notice prior to termination of eligibility for Medicare Benefits. When
admitted, the Medicare patient will receive a bulletin, "An Important Message for Medicare
Patients," which explains the rights of the Medicare Patient.
Patients have the right to be informed of Hospital policies and procedures applicable to their
conduct as patients.
Patients are entitled to information about the hospital's mechanism for the initiation, review, and
resolution of patient complaints.
